Please download the desired 2012 education form. Please download the form first, complete the form, and email the form to me. You may pick up your books 2 weeks before the class. At that time, please provide the checks made out to MVRMC.

If you are not an employee, please make one check for the cost of the course and one for the rental of the book. If you are an employee, please make one check out for $20 to hold your spot and one check for $30 to rent the book. Please keep in mind that you will need to bring $1 for the card.

If the class is a recertification class, please bring your current card with you.
