Welcome to the Orientation home page. We have activated the student orientation link. In the future, we will also have an RN contract orientation link identified on the orientation home page as well.
Under the student orientation link, please open and save all the documents to your PC. Please start with the checklist. That will be your guide to the requirements for student orientation. When you have completed the videos, the packets, and the tests/signature pages, please check completed on the checlist and sign the checklist electronically. Please email the checklist and all the tests and signature pages back to Gail Martinez. My email address is listed at the bottom of the page. Annual Reorientation
CHS requires that all employees have an annual reorientation. Employees need to have their reorientation completed on or before November 30th each year. Employees should have already completed the following courses thru Advanced Learning Center (ALC).
- 2010 Annual compliance Training (signed form to be returned)
- 2010 Code of Conduct (certificate to be returned)
- 2010 General Compliance Training
- 2010 HIPAA Basic Overview (certificate to be retuned)
In addition, the following forms will also need to be reviewed and have signed. They are on the MVRMC web site, under Orientation Tab (you are here) as well:
- Annual Personal Disclosure
- Missed Meal Acknowledgement Form
Please read, complete, and turn-in to your Director or Manager.
